I think you may have the medic trade confused. They're military versions of paramedics, the guys who arrive and help you in an Ambulance.

I personally wore a shirt and tie when I got sworn into the Reserves. From what I understand, it's really up to personal preference and common sense.

What are some of your favorite Armed Forces movies? It doesn't have to be super realistic, but not as crazy as Starship Troopers. I just saw The Hurt Locker the other day, I thought it was pretty baddass. Saving Private Ryan, Black Hawk Down, and Full Metal Jacket all kicked ass in my opinion.
