    Call the CFRC and ask would be my first suggestion. My *guess* would be that the form needs to be filled out to request your old med documentation from Ottawa/wherever it is located since you were a prior member,  but this is just a guess (I don't recall filling one out myself).

    Congrats... Im still hoping/waiting on SIP numbers for CEOTP for 2008/09....  (Civilian Competition). BMOQs (IAP/BOTP) traditionally would have its next run in May....  but they have been playing with courses a lot from what I hear so... who knows.
