Laser eye surgery admin requirements

Have the admin requirements been relaxed? Can anyone in the medical/admin or command comment to this. I have noticed several people have the treatment but know of only one who had received permission via request form and memo.

My interest is that I intend to have the procedure myself and want to adhere to the current regulations.

My surgery was a year ago, and I asked the PA in theatre what I needed to do before I got it. Basics are, memo to your CO requesting permission. Outline when the surgery date is (or tentative date), any time off (probably need to use annual unless you do it on a Friday), and that you are willing to accept the responsibility should the surgery render you unable to meet Universality of Service. Once I got my vision stable, I got a form from the MIR, had it signed by my civvie laser eye surgeon, booked my Part 1 and Part 2 medical, and had my V cat changed. From surgery to med cat change was ~3 months, your results may vary.
