Letter Formatting

I have to send a letter to DMC3.  I know there is an official letter format in the wonderful big book of military writing (I saw said manual and used it more than once while I was in)  Can some please send me a copy of the format so I can get this written up and have it look professional.  The sooner I get this done its possible the sooner I can get back in and we all know how short the CF is on trained ACISS members.
