Im looking for the document that we fill out to tell the CF were to deposit our pay. The  one were we join a check.

Any one can help ou?

Check with your pay office.  Anywhere I have ever been, it was only a locally produced form where they took your particulars, and a space to fill in the banking information if you didn't have a void cheque.
