Another mishap plagues an RAN Collins class sub:

Defense Industry Daily

Feb 27/14: HMAS Waller. The submarine HMAS Waller, fresh out of scheduled maintenance, experiences a fire while traveling on the surface. Nobody dies or is injured, but 4 sailors are helicoptered off for observation. The Navy is still looking into the cause. Sources: Australian DoD, “Fire onboard Royal Australian Navy submarine”.
 
and a fire on a Indian sub, 2 missing and I think 7 injured, subs are always going to be a fairly risky business.
 
Agree with Colin: Submarine business is always a risky business.

Moreover, I would surmise that right after coming out of maintenance is probably riskier rather than safer. No matter how careful one is during the post maintenance inspection, it is still easy to miss something small or hidden that works or the inspection but gives you a headache quickly thereafter.
