Tubes tied or to have the Essure procedure done. Do the military doctors do it, or are you sent to a specialist? What about IUDs like Flexi-T, which are becoming quite common? Do you generally have to fight with the doctors get permanent or semi-permanent sterilization done or to be referred? My IUD expires in 2012, I'm hoping to get the Essure done, if possible. I want to know what to expect or how to prepare myself to not get shot down.
 
A doctor, or specialist, who may or may not be in the employment of the CF will do it if your are in the CF. Go see your medics/CDU/clinic, and they will be able to specifically answer the question.
 
As you are quite young (if your profile info is correct), you may have a hard time getting permanent sterilization done.  Is that fair?  IMHO, no, but that's the way it is.
An IUD should be no problem.
 
I'll go out on a limb here and guess that you're a serving member? That's the downside of an empty profile. We can only guess the answers to a supposed situation. This makes it doubly difficult because we don't know your situation specifics and we're not doctors.
If you are serving, make an appointment with the MO and discuss it with them. They can answer all your questions regarding this matter. If you're a civvie, go see your preferred OBGYN.
 
I'm soon going to be a serving member, I'm currently a civi. The way it worked for me before, was in order to get to see a gyno, I had to make an appointment through a GP or planned parenthood. I don't have a GP. Planned parenthood set me up with a contraceptive specialist, but it took two months to see her for the initial appointment and then another month to get the IUD insert. I still have four years until my IUD expires and by then I'll be a serving member.

When you are serving you will have a MO, PA or NP who will be able to refer you for the treatment you require. No worries about that. Get in, do what you need to do, and 6-8 months or so away from your expiry, start inquiring with your units medical support.
