Afghanistan medal/bar for multiple tours?

I'm trying to find out information on the bar (I guess it's called?) for members who have multiple tours to Afghanistan. A few guys I know might make the criteria so I'm checking for them.

How many days do you require in country to be eligible for the bar?
Would they just submit a request through their OR?
Where might a reservist check to see how many days they were in country, MPRR or something?

30 Days in theater get you the medal, and then every 180 days after that you are eligible for the bar.

210 days = 1 bar
390 days = 2 bars
570 days = 3 bars
750 days = 4 bars

180 days more, for your 5th, you get one bar with 5 maple leaves on it, and then a bar for each 180 days after that

Application is made, via the soldier's chain of command, through the OR; your CClk should know the procedure from there.

Res, like RegF, check their MPRR.

If the dates are accurate.  I had to have the clerk check through the pay office.
 
For released members, is there a procedure to get these bars? Looks like I would qualify for two of 'em.

Happy to see this change. I recall of guys were very displeased with the certificate instead of a medal after their second (or more) tour.

What's funny is that before coming to this site right now, I was just on the phone and my gf was asking me if I had called about getting my bar yet.. Guess the fact this is right here is a sign, right? lol

Your OR can email the DHR clerk (I have the name if you need it) and they will check your entitlement and order it if you're supposed to have one. Currently, its a 6 month to 1 year wait for rotation bars.
 
...although your mileage may vary -- my bar showed up in about 6 weeks; sheer luck, I assume.
 
Since there seem to be knowledgeable people in this thread, about how long does it take to get a medal with a spelling error reissued?
 
it shouldn't take too long as there is no further research required.  Contact DH&R through your chain of command.

Took me the better part of a year letting the unit I was attached to (CIMIC) handle it and they never sorted it. My home unit had it handled in about three weeks.
