On June 25, 2019, Katalin Melnik, a 74-year-old Arizona grandmother, fractured her nose while hiking Piestewa Peak in Phoenix and reluctantly agreed to a helicopter evacuation by the Department of Public Safety. During the lift, a detached guideline caused her rescue basket to spin uncontrollably for 30 seconds, resulting in minor additional injuries like whiplash and bruising, from which she fully recovered.
