I did my medical last Wednesday and at the end they gave me 3 papers to get filled in. One for my optometrist (about my glasses prescription), one for my dermatologist (because I am prescribed antibiotics for acne) and one from a doctor (about a previous ADHD diagnostic).
The first too I had no issues with but the third one is a different story.
2 years ago I was diagnosed with ADHD by a psychologist I went to see about lack of motivation in school that was causing me to have issues concentrating. She convince me I had ADHD and strongly suggested I took mediation. I got some prescribed by my old pediatrician (I was 17 and did not have a family doctor) took them for about 2 months and stopped because It didn't change anything and had bad side effect.
I went through the big majority of high school without meds (only 1 month with) and got good grades, Held 2 summer jobs without meds and did just fine and now I'm in college without meds and doing good.
The paper I have to get filled in is basically to say if my ADHD affects my life when I'm off meds and if I'm fit for military service. I have not had any follow up about the ADHD thing for 2 years so nobody can really know how I am currently doing. I went to my old pediatrician and he told me that since he did not see me while on meds he cannot really tell if I'm better with or not. He wants the report from my psychologist, which obviously says I need meds. Like I said I'm doing so good without them and always have so I don't get it...
The lady at the medical said ''get this filled in by your doctor or if you don't have one then in a CLSC''. I don't have a doctor and the people at the CLSC says I couldn't get it filled there since no doctors there know me enough for that (which makes sense)...I don't know what to do.
