how to fill in the "personnel screening consent and authorization form"?

i see some parts you need authorized departmental/agency/organizational officials to fill it in

where do i find these people?

They fill it in when you submit your application. The reasons they have multiple agencies/authorized personnel, is because your screening may be done at a local level or they may have it screened by CSIS our outsourced. You don't have to fill out that portion.
