The 251 is an older built Esci/Ertl repaint and the figures are commercially available. IIRC the Div CO himself is from the OOP Hasegawa 234/1 kit.
The ladder on the side can be seen in many period photo's of command tracks. A quick guess would say it is an easier way to enter the track than trying to squeeze past the radio operators and their radio's. (I made the radios from styrene and painted them grey with some white dots. Did not pick up in the pictures..)
