Good Day All,
  Forgive me if this question has been answered already, as I failed to see it! When completing an application for hearing loss, does the form PEN 57E (Medical questionnaire - Hearing Loss/ Ear conditions) have to be completed and attach to the application?

Yes.

Print it off and ask your audiologist to complete it. Submit it with your disability benefit application.
