The doctor and the CFRC gave me note,for my family doctor to fill out

when I completed all my tests the doctor gave me a not to be filled out by my doctor. It was concerning my heart surgery I had when I was one. WIll he want my medial records, or just that note once it was filled out? ANy answers would be very appreciated. Cheers!

You really should ask him what he wants, but I was given a form for allergies and they just needed the form filled out by a doctor.

My Dad is living back home, and he is getting my medical records, just in case, so hopefully that will be enough. Cheers.
