Brake lights are activated by an air pressure switch usually set between 2-5 psi. If there was an air pressure issue than the brake lights would not light up. Again extremely unlikely because when the air pressure falls that low the park brakes automatically engage.
As a Heavy truck mechanic brake failure on an air brake system is HIGHLY unlikely. I have not seen it on a truck in my 20+ year career. More likely she didn't brake or she missed the brake pedal and hit the throttle instead. Why did the crown not retrieve the onboard computer data?

Most three lane sections of the 401 trucks are not allowed in the left lane. there are some places where they are allowed. Speed limiters at 105 kph are law in Ontario but enforcement has been lax and many trucks have them higher now.
