Pre-Enrollment forms (NOK and Emergency Contact Info)

My BMQ starts next Monday. (Enrollment is the Friday coming up (13th) and I've just begun to look at the forms that I need to fill out to bring to my enrollment. I've got a couple questions, if someone familiar with these forms could assist me..

On both forms, Section 1 has a 'Members Information' section. Is this something that I fill out? It has boxes labeled -
SN-NM (Don't know what that is)

On the Next of Kin form  -
It also has a box for SN-NM

I'd appreciate it if someone could help me out with (If) what I need to fill out for that section.

SN is Service number.  It is a Letter followed by 8 numbers i.e. A12 345 678 it is a unique number assigned to ever person in the military if you have not been given one yet you will be.  Rank your rank will be Pte (if army).

As with any paperwork if you are unsure of anything leave it blank and bring it to the attention of someone and they will assist you.
