So when you join as a medic with the reserves. If you want to say specialise in psychology...is that an option? or possible?

I think you may have the medic trade confused. They're military versions of paramedics, the guys who arrive and help you in an Ambulance.

No.  Not even in the Reg F do Med Tech do this.

The U.S. Army does.  Not Canada. 
The reserve field ambulances are able to recruit other specialists, but you need to be already
trained from civilian life or coming from the full-time regular force.


 
This is dated from a few years ago, so it might have changed sicne then but it is my understanding (from a psychologist working for the CF) that all psychologists working for the CF are civilian practitioners who are experienced in their field and are hired by the military to fulfill temporary needs or a contracted period of time. 
 
PSO's generally have to have psychology degrees as well IIRC.

The occupation within the CF most populated by clinical psychologists are, as MM mentioned, Personnel Selection Officers. Otherwise they are employed as civilian practitioners on a contractual basis.
