I've seen it done, happens far more then people realize. Mind you the system may work but it's very close being overwhelmed.

I have formed people before - by going before a judge and swearing an affidavit under the MHA Form 2...and the warrant was issued. Now, I do it Form 4 but under someone else's name/license when in clinic since I'm in a remote area, and 9/10 times after consulting a psychiatrist, with a few exceptions of course. Literally suspending someone's civil rights for 72 hours. Was a hair's width from it today - lucky I got a consultant head crusher to see them via telehealth...and to my surprise said "Meh, let them go and let them be for now..." I'm hoping I'm not going to have to quote them on that at 0 Dark OMFG tomorrow morning...

Is that for involuntary assessment or admission (assuming Manitoba)?
