What insignia does the good Commander wear on his shoulders?

ADCs don’t wear them unless the are actually on ADC duties.

It is not surprising that dress regulations seem to be at odds with actual practice (or is it that practice is at odds with regs). Looking through the applicable section, the good commander seems to be following common practice (shoulder straps added to the jacket) that would be aligned with dress instructions.
Officers below flag/general officer rank shall wear cloth shoulder-straps on ceremonial, service and mess dress jackets, on which the cyphers and badges are authorized to be worn. On jackets which are not normally equipped with shoulder-straps, plain shoulder-straps of the pattern worn by general officers, less badges of rank, shall be felled onto both shoulders, at the shoulder seams, and shall be fastened at the collar ends by means of 26-ligne buttons.
But then it directs to an annex for instructions on how to wear.
  1. Aiguillettes, Royal cyphers and personal badges shall be worn as noted in Appendix 1 to this annex.
  1. Navy – Right Shoulder Pattern. When holding a non-permanent personal appointment, the right-shoulder ceremonial aiguillette shall be attached to a gold plaited shoulder cord, which in turn shall be secured to the right shoulder of jackets not otherwise equipped with shoulder-straps. When shoulder straps or boards are worn, the aiguillette shall be attached under the right shoulder-board or strap.
That appendix illustrates that attaching aiguillettes on navy dress is different that for the other services.

Traditionally, only one shoulder cord is worn, depending on the principal that one is attending on, as can be seen in these photos of visits from the late Queen with Prince Phillip (attired as a Canadian Admiral) and the now Prince of Wales. Note the single gold corded shoulder strap on the equerries.

Though at least one Canadian VAdm didn't get the memo about shoulder straps.

The only time that shoulder cords should be used on both shoulders would be when one was wearing the sash of an order or a collar chain.

I surveyed examples of Navy AdeC to Lt Gov for a few provinces and most appear to follow the example of Manitoba. A few odd individuals wore what appeared to be a mix, but Nova Scotia appears to be consistently aligned to the appendix to Annex G. These photos, from the last couple of years show them wearing the right shoulder cord and aiguillette with the appropriate provincial badge on only the right shoulder.
